HD14-SBSUB prevent subwoofer failure - Advisory

My aftermarket saddlebag subwoofers (HD14-SBSUB) sound very distorted or failed easily.  How can I fix this? 

This HD14-SBSUB issue can be caused by the installer plugging the subwoofer amplifier's input into the primary amplifier's speaker outputs.  This is essentially amplifying the signal, then amplifying it a second time with the subwoofer amp.  To fix this, the subwoofer input harness should tap directly into the radio outputs.  This is done with a Y-adaptor at the primary amp's rear input terminals.
